I pierced my fraenum yesterday. My friend did his a while back and the ring was torn out as he slept. I threaded my glans ring through my piercing and then looped the ring over my glans (cock head) with the ball next to my fraenum. Hopefully that will protect it. It nestles under my foreskin and has the added advantage of holding my foreskin back.

The skin of the frenum is very thin so you need a reasonably heavy gauge of metal to stop it tearing through. At least 14 Gauge or even 12 gauge is ideal. Anything less and you may have problems.
I have had my frenum piercing for 10 years now with no problems at all despite some very vigorous activity! :)
