Your sphincter is fairly thickened by now if you have regular anal sex. the rectum is much bigger and when lubed the penis doesn't interact much with the lining, unless you have and odd curve or he is massive. The tissue is also very expandable. Keep in mind men get fists, very big toys and all manner of much more severe toys in and come through fine. If you have had pinking or bleeding in the past right after or longer this may be a burst capillary (pinking) not a huge risk and a thing that happens many places in the body or you popped off a hemorrhoid. When volume is low and it stops fast there is no worry. The penis did what the proctologist would. Its scary to experience but unless profuse and ongoing not a health emergency. The dots will cause more friction and mild stretching but I can't see how they would injure you.
