I threaded my Glans ring through it and then looped it over my glans (cock head) to stop it pulling. The ball is right next to my fraenum. It has the bonus of holding my foreskin back - something I particularly like.

I forget about the ring tightening as I get an erection (I'm a grower so my cock swells a lot). It can be very painful, especially with the morning wood. So now I have fritted a slightly looser ring.
